Maintenance Workshop Registration Form
Fees must be received in full with your Registration form to be confirmed. Each seminar has a limited seating and will be filled on a first come first served basis with priority given to users of National equipment. There must be a minimum of 6 participants for a workshop to be held. Confirmations will not be sent until this minimum is met. If there are not enough participants, you will be notified e-mail.
Full refunds will be issued for cancellations made up to two weeks before seminar begins. Cancellations received less than two weeks before the seminar are subject to a $150 administrative fee. Confirmed participants who do not cancel and do not attend are liable for the entire seminar fee. Confirmation of your attendance will be made to the first contact address or email, fax or phone number listed on your registration form, not your mailing address. A substitution can be made at any time and should be submitted in writing.
IN THE WORKSHOP FEE: The fee includes attendance for one person, all workshop materials, three lunches at National, and dinner the evening of the first day of the seminar. Travel and living expenses are not included.
MEETING DETAILS: All sessions are held in our plant at 161 Greenfield Street, Tiffin, Ohio. They will begin promptly at 8:00 AM on the dates indicated and end at approximately 4:00 PM the last day.
